You will receive an appointment confirmation message via text including a link to the Patient Registration forms above. Please complete registration online prior to your first appointment. If you prefer, you can easily print and complete all forms from here and bring with you to save time at check in.
If, for any reason you cannot keep a scheduled appointment or will be delayed, please call Metropolitan Dermatology as soon as possible.
Financial Policy & Insurance Billing
We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
